(A) Proof of Vaccination
Please see below samples of COVID-19 vaccination proof to be uploaded. Please make sure your name (first letter of each word if you would like to mask it) and the date of your first dose can be clearly seen in your screencap.

(A) COVID-19 Testing Results (for no vaccination only)
The Centennial Gala is open to both vaccinated as well as non-vaccinated guests. For non-vaccinated guests, please select “PCR/RAT test” in the dropbox. Please upload your PCR/ RAT test screen on or before 23:59pm 18th Nov. We will accept 1) PCR test to be done 7 days before 2) RAT test to be done 3 days before the Gala show.

(a) PCR – a picture of your test results (letter, screencap or any other means). Please mask and show us only (a) the first letter of each word in your name; (2) last 4 digits of your HKID number
We accept PCR tests results in any laboratory or test centers.
(b) Rapid Antigen Test (RAT) – a picture of your test kit that shows negative. We accept RAT kits from any manufacturers. RAT kits can be easily bought in Watsons stores located throughout HK.
